Telephone Distribution Program (Provided through Florida Telecommunications Relay, Inc.)
Eligibility is open to anyone who is a Florida resident who is deaf, hard of hearing, deaf/blind (visually impaired) or speech impaired. In order to receive specialized telephone equipment you must provide proof of eligibility.
There is no charge for FTRI or Relay services. You must also provide proof of residency with one of the following: Florida Diver's License, Florida Voter Registration Card, Florida Identification Card, Florida Vehicle Registration, Florida Medicaid Card, Letter from Social Security with your name and Florida address, letter from Florida nursing home

Hours for pick-up are: Walk-ins - Tuesday & Thursday from 9:00am to 12:00pm and 1:00pm to 4:00pm
By appointment: Monday, Wednesday, and Friday. Please call the Center for alternative distribution sites or educational presentations and group remote distributions can be arranged.
Offsite distributions: If your organization is interested in distribution of specialized phones for their members or residents, please contact us at 954-601-1930 or agoldman@chchearing.org.
